Analysis Application Stages and Process




1. APPLICATION AND SAMPLE ACCEPTANCE PROCEDURES

a) Individuals, private institutions, public institutions and organizations wishing to have an analysis performed shall fill in the Analysis Application Form and the Analysis Request Form prepared separately for the analyzes they wish to have performed.

b) The relevant forms are sent directly or via various means of communication (e-mail, fax, cargo or mail) to the Central Directorate, whose contact information is given below.

c) After the application is evaluated by the experts of the Central Directorate, the samples are accepted for analysis. If deemed necessary, Material Safety Data Sheets (MSDS) are also requested for the samples.

d) Samples must be packaged in sealed glass or plastic containers in a manner that does not harm human health and the environment. Sample containers must have a label with descriptive information about the sample (such as the name of the person/organization, sample name, quantity, sampling date).

e) Samples must be delivered separately for each device and must be in the quantities defined for that device. For this reason, it is advisable to discuss the quantity with the relevant laboratory unit before sending the sample. Samples that do not comply with these conditions are not accepted for analysis

2. PAYMENT OF THE ANALYSIS FEE
a) Pricing is made by the Central Directorate for the samples accepted for analysis and the amount including taxes is notified to the person or institution requesting the analysis.
b) After the person or institution delivers the receipt of payment of the analysis fee to the bank account number to be obtained from the Central Directorate to the Central Directorate, the analysis procedures are started.
c) For analysis and measurement services that are not included in the price list published by the Center Directorate every year, a separate offer is made to the person or institution requesting such services.If the offer is accepted, analysis procedures are initiated.
d) For urgently requested services, a 70% price increase is applied in case of working out of working hours and/or on a holiday, and a 50% price increase is applied for works prioritized during working hours.

3. REPORTING AND INVOICING
a) The report and its results may not be used for commercial and advertising purposes by the requesting organization and its customers. The report cannot be reproduced / published in whole or in part.

b) For the analyzes requested in the application petition, only a report showing the analysis results is issued. If a separate report is required for more than one sample, this must be indicated in the Application Form.
c) Separate fees are charged for special report requests that include the interpretation and evaluation of the analysis results by expert faculty members. Fees are determined by the Center Directorate according to the nature of the work.

d) The invoice is issued using the information provided by the applicant person or institution in the Application Form. Analysis result reports, invoices and samples remaining from the analysis are sent to the address specified in this form by cargo with payment or delivered by hand.
